Current models of healthcare fail to serve those in most need like those most vulnerable to adverse health conditions like the elderly or pregnant women and those living with chronic communicable or non-communicable disease conditions, or most rural populations. In many parts of the world, the health literacy of the general population is too low to pursue effective self care or there aren't enough resources available for that. As such, healthcare beyond health centers and hospitals is a must. To seamlessly integrate such healthcare extension programs or community-based healthcare programs to facility-based healthcare, we need to develop innovative programs like outreach programs to cater to rural areas, emergency preparedness, referral loops for chronic conditions and an information-system that integrates mHealth (in the community) and Electronic Medical Records (in the health facilities).
